Are You Wasting Time with IUI? Know When to Choose IVF

Patient: Dr. Malpani, my gynaecologist, has recommended IUI for me. What are your thoughts on it?

Dr. Malpani: That’s a great question. As an IVF specialist, one of my biggest frustrations is the overuse and misuse of IUI treatment. Many gynaecologists who don’t offer IVF often recommend IUI to all infertile couples because it’s the only treatment they can provide. Unfortunately, this one-size-fits-all approach can do more harm than good.

Patient: Really? Why is that?

Dr. Malpani: Think of it this way—when all you have is a hammer, everything looks like a nail. IUI can work in certain cases, but it’s not effective for everyone. For example, if the sperm count is low, IUI is unlikely to succeed. Yet, some doctors still insist on trying it, often making exaggerated promises.

Patient: So what happens when it doesn’t work?

Dr. Malpani: That’s where the real problem begins. Patients often move from one doctor to another. First, they do a couple of IUI cycles with Doctor A, then try two more with Doctor B, and another set with Doctor C. By the time they’re fed up and frustrated, they’ve wasted a lot of money, lost confidence in doctors, and, most critically, lost valuable time.

Patient: How does that impact the chances of success?

Dr. Malpani: Time is crucial, especially for women. With every failed cycle, ovarian reserve can decline. By the time some patients are ready for IVF, their chances of success are lower because their ovarian reserve has diminished significantly.

Patient: That’s disheartening. How can patients avoid this situation?

Dr. Malpani: The key is to do your homework and ensure you’re receiving the right treatment for your condition. Understand whether IUI is appropriate for you or if IVF is a better option from the start. A well-informed patient is an empowered patient, and that makes all the difference.
